Accommodations for 10 nights (including all hotels and the resort).
All activities and sightseeing mentioned in the itinerary (including yoga asana classes).
Meals mentioned in the itinerary (ie breakfast and almost all lunches and dinners).
Transport between locations mentioned in itinerary (ie van, shuttle, taxi, tuk-tuk, flight from Bangkok to Chiang Rai, VIP bus from Chiang Rai to Chiang Mai).
Not Included
Flights/Airfare to the starting point of your yoga tour and after the end of the tour.
Transport (ie taxi) from airport to hotel (first day) and from hotel to airport (last day).
Excess baggage charges, fees for travel documents (passports, visas, etc.), travel insurance, alcoholic beverages.
"On your own" activities and meals in the itinerary, gratuities/tips and personal expenses (ie souvenirs).

Ideas for what to pack
Consider bringing lighter clothes as it will be somewhat warm and humid and a cap or hat for the sun. You might want to bring a thin rain poncho and/or umbrella, though most hotels and the resort will provide umbrellas and you can buy the poncho locally too. Note also that knees and shoulders are to be covered for modesty when entering temples per Thai custom. So, a thin shawl and long cotton pants could work. Also, bring power adaptors for Thailand (ie 3 for $10 on Amazon) for charging your laptop, phone, etc. Bring an old pair of shoes that could get wet/dirty at the elephant sanctuary and for trekking. Sandals, sunglasses, sunscreen and natural mosquito spray if you'd like. Finally, bring your yoga mat and yoga bag to carry your yoga mat (so you don't have to buy or rent/borrow a used one).

Money, passport and visa
It's a good idea to bring a credit card with "no foreign transaction fees" and an ATM card. There are many "currency exchange" booths at the airport in Bangkok, but another option is to use your ATM card to take out cash (Thai Baht) from an ATM machine while you're waiting for your bags in baggage claim. There's a small fee, but it's a very convenient and easy option. Having some cash will come in handy for tips, souvenirs, street food, etc. Also, make sure your passport isn't expiring within 6 months of the yoga tour dates and make a copy of the picture/info page and keep it with you separately, just in case. Finally, if you have U.S. citizenship, you are eligible for a visa waiver for up to 60 days for tourism. You don't need to go to "Visa on Arrival" or pay anything. It's automatic.

Ride to and from airport
Consider downloading an app for a ride to and from the airport. Uber and Lyft are popular in the U.S., but aren't used in Thailand. (Note: Uber sold its operations in SE Asia to Grab.). So, one app to download is Grab. Grab is probably the most popular and well-known. Then, you can request a ride very simply. Another option may be to get a taxi at the airport. Again, it's a good idea to have cash on you before leaving the airport. (Note: As far as cash tipping, usually a tip of 50 Thai Baht for the ride is appreciated, and a tip of 100 Thai Baht, which is about $2.50, (or more) is especially appreciated.)
